About the Role:
The role is for a full-time Nurse Practitioner at Hamilton Park Nursing and Rehabilitation Center in Brooklyn, NY. The ideal candidate should be clinically confident, compassionate, and autonomous with experience in SNFs, home care, or hospital-based geriatric care.
Key Responsibilities:
- Deliver primary and urgent care in person to SNF residents.
- Evaluate, diagnose, and manage chronic and acute geriatric conditions.
- Collaborate with facility staff (nurses, doctors, social workers).
- Comply with EHR documentation standards and state regulations.
- Educate patients and families about care plans and wellness strategies.
- Apply evidence-based medical practices to improve outcomes.
Qualifications:
- Active NY State Nurse Practitioner license
- Minimum 3 years of clinical experience in relevant settings
- 3,500 hours of NP practice
- Valid NPI number
- Excellent communication, documentation, and EHR skills
- Passionate about elder care and value-based medicine.
