About the Role:
The client is currently seeking an experienced In-Person Nurse Practitioner to join the team at Hamilton Park Nursing and Rehabilitation Center in Brooklyn. This is a full-time, W-2 position ideal for a clinically skilled NP with a strong background in SNF, home care, or hospital-based care. The ideal candidate will be confident in autonomous practice, compassionate, and well-versed in managing chronic and acute conditions in older adults.
Key Responsibilities:
- Provide in-person primary and acute care to SNF residents.
- Perform routine evaluations and manage chronic conditions.
- Conduct assessments, develop treatment plans, and document all encounters.
- Collaborate with interdisciplinary facility staff including physicians, nurses, and social workers.
- Ensure compliance with regulatory standards and documentation requirements.
- Educate patients and families on care plans, medications, and wellness strategies.
- Utilize evidence-based practices to improve patient outcomes.
Qualifications:
- Active Nurse Practitioner license in New York State (required)
- Minimum of 3 years of clinical experience in SNFs, home care, or hospital settings
- 3500 hours as a Nurse Practitioner.
- Possession of a valid NPI number.
- Strong clinical skills and the ability to work independently
- Proficiency in medical documentation and EHR systems
- Excellent communication and interpersonal skills
- Passion for elder care and value-based clinical practice
